Autistic and Hyperactive: Autism, ADHD or Both?

Can autistic people be hyperactive? Yes. An autistic person can appear restless, constantly move, seek stimulation or struggle to sit still. But hyperactivity does not automatically mean someone has ADHD.

Sometimes movement is connected to autistic stimming, sensory regulation, excitement, anxiety or overload. Sometimes it reflects co-occurring ADHD. And sometimes several of these processes are happening at once.

This is particularly important because autism and ADHD frequently overlap. They are separate neurodevelopmental conditions, but a person can meet the diagnostic criteria for both. When autism and ADHD occur together, the combination is often informally called AuDHD.

So instead of asking only, “Is this hyperactivity autism or ADHD?”, it can be more useful to ask:

What is driving the need to move, seek stimulation or switch activity?

Can autistic people be hyperactive?

Yes.

Hyperactivity is most strongly associated with ADHD, but autistic people can also show behaviors that look hyperactive.

This might include:

⚡ constantly moving
🦵 bouncing a leg
🚶 pacing
🪑 struggling to remain seated
👐 moving the hands repeatedly
🗣️ talking rapidly or intensely
🔄 moving repeatedly between activities
🎧 actively seeking sensory stimulation
🎯 becoming extremely animated around an interest
🌀 feeling physically restless when overwhelmed

The outward behavior may look similar.

The underlying mechanism may not be.

An autistic person pacing around a room because repetitive movement helps regulate overwhelming sensory input is experiencing something different from someone whose ADHD makes remaining still extremely difficult across many situations.

And one person can experience both.

Is hyperactivity a sign of autism?

Hyperactivity itself is not one of the defining diagnostic criteria for autism.

Autism is characterized by differences in social communication and interaction alongside restricted or repetitive patterns of behavior, interests or activities. These can include repetitive movements and unusual responses to sensory input.

That creates an important source of confusion.

A behavior such as:

pacing back and forth

could potentially reflect:

🔁 autistic repetitive movement
🎧 sensory regulation
😰 anxiety
🔥 overload
⚡ ADHD hyperactivity
🧠 several mechanisms simultaneously

You cannot reliably determine the cause from the movement alone.

Context matters.

The NICE guideline for autism in adults similarly emphasizes assessing the broader context around behavior, including sensory factors, communication, physical health, mental health and changes in routines or circumstances.

Autism vs ADHD hyperactivity

One useful way to understand the difference is to ask what function the movement serves.

ADHD hyperactivity

In ADHD, hyperactivity is part of the core diagnostic profile.

It can involve:

⚡ persistent physical restlessness
🦵 fidgeting
🚶 difficulty remaining seated
🗣️ excessive talking
⏩ feeling driven to keep doing something
💭 difficulty tolerating inactivity
🎯 seeking stimulation
🧠 an internal sense of restlessness

In adults, hyperactivity does not necessarily look like someone literally running around the room.

It can become more subtle.

An adult may constantly change sitting position, fiddle with objects, tap their feet, talk rapidly, keep themselves continuously occupied or experience a persistent internal motor that rarely seems to switch off.

Autistic movement

Autistic repetitive movement—or stimming—can look very similar from the outside.

Someone might:

🔁 rock
🚶 pace
👐 flap or move their hands
🦵 bounce their legs
🎵 repeat sounds
🖊️ manipulate objects
🌀 spin or move rhythmically

But these movements may help regulate sensory input, emotion, attention or arousal.

An autistic person may also become much more physically active when excited about an interest or when trying to regulate overwhelming input.

The behavior is therefore not necessarily ADHD hyperactivity.

Same movement. Different possible function.

Stimming vs hyperactivity

This distinction deserves particular attention because stimming is frequently mistaken for hyperactivity.

Imagine someone repeatedly bouncing their leg.

From the outside:

They cannot sit still.

But internally, several things could be happening.

🎧 Sensory regulation: The movement provides predictable sensory input that helps filter an unpredictable environment.

🧠 Attention regulation: Moving makes it easier to concentrate.

❤️ Emotional regulation: Repetitive movement helps manage anxiety or excitement.

Hyperactivity: Remaining physically still feels intensely uncomfortable.

🔥 Overload: Movement is helping discharge mounting nervous-system activation.

Or perhaps:

all of the above.

This is why suppressing movement simply because it looks unusual can be counterproductive.

If a movement is harmless and serving an important regulatory function, removing it without understanding its purpose may make concentration or regulation harder.

Sensory seeking can look like hyperactivity

Autistic people can be hypersensitive to some forms of sensory input while actively seeking others.

Someone might dislike unpredictable noise but crave:

🌀 spinning
🏃 intense movement
🎧 loud predictable music
🤗 deep pressure
🦘 jumping
🪑 rocking
💪 heavy physical activity
🔁 repetitive movement

This is sometimes called sensory seeking.

A sensory-seeking autistic person can therefore look extremely active.

They may continually move because movement itself provides the sensory information their nervous system is seeking.

This can seem contradictory if autism is imagined only in terms of sensory sensitivity.

But sensory processing is more complex than simply being “too sensitive.”

Someone can be highly sensitive to one kind of input while under-responsive to another.

Hyperactivity during sensory overload

Movement can also increase when an autistic person becomes overwhelmed.

Imagine spending an hour in an environment containing:

🎧 multiple conversations
💡 bright lighting
🚶 people constantly moving
📱 notifications
👃 strong smells
🗣️ social demands
🔄 unexpected interruptions

As the total load increases, regulation may become increasingly difficult.

Someone might start:

🦵 bouncing their leg faster
👐 moving their hands
🚶 pacing
🎧 seeking predictable sensory input
🗣️ talking more rapidly
🔁 repeating movements

To an observer, the person suddenly looks more hyperactive.

But the change may actually be an early sign that sensory and cognitive demands are approaching their capacity.

Recognizing that pattern can be useful because it shifts the response from:

“Stop moving.”

to:

“What is making regulation harder right now?”

Can you have autism and ADHD together?

Yes.

Autism and ADHD are separate neurodevelopmental conditions, but they can occur in the same person.

This is now well established. Research in adults repeatedly finds substantial overlap between autistic and ADHD characteristics, while also showing that the relationship is complex rather than simply one condition looking like the other.

A 2024 multi-method study specifically examined the overlap between autism and ADHD characteristics in adults and found both shared and distinct patterns. Read the study in Cortex.

Studies of adults diagnosed with ADHD have likewise found meaningful autistic characteristics in some participants, reinforcing the importance of considering both conditions during adult assessment. A 2022 study using the ADOS-2 found that apparent autistic characteristics in adults with ADHD could reflect different underlying mechanisms.

When someone meets diagnostic criteria for both autism and ADHD, the combination is commonly called AuDHD within neurodivergent communities.

AuDHD is not a separate formal diagnosis. Clinically, the person receives diagnoses of autism and ADHD.

What does autistic hyperactivity look like in AuDHD?

When autism and ADHD occur together, the person’s needs can sometimes appear contradictory.

They may experience:

🔄 Autism: I need predictability.
ADHD: I need novelty.

🎧 Autism: This environment contains too much stimulation.
🧠 ADHD: My brain needs more stimulation.

📅 Autism: Routines help me function.
🔥 ADHD: Maintaining routines is difficult.

🎯 Autism: I want to stay deeply engaged with this interest.
🦋 ADHD: My attention keeps being pulled somewhere else.

🏠 Autism: Familiarity feels regulating.
🌍 ADHD: Too much sameness becomes unbearable.

This does not mean every AuDHD person experiences these exact contradictions.

But they illustrate why looking at autism and ADHD independently can sometimes fail to explain the whole experience.

Someone can simultaneously be overstimulated and understimulated.

Can autism look like ADHD?

Yes, some autistic characteristics can resemble ADHD characteristics.

For example:

This overlap can complicate adult identification.

Research comparing adults with ADHD and autistic adults has found overlapping observable characteristics while also identifying differences between the groups. A 2025 ADOS-2 comparison study provides a useful example.

The key point is that similar behavior does not necessarily mean identical underlying processing.

Can ADHD look like autism?

The reverse can happen too.

An ADHD adult may:

🗣️ miss social cues because attention shifted
👥 interrupt conversations because of impulsivity
📅 depend heavily on routines to compensate for executive difficulties
🎯 hyperfocus intensely on particular subjects
🎧 become overwhelmed in busy environments
😰 struggle with social situations after years of negative experiences

Some of these behaviors can resemble autistic characteristics.

Research has found measurable autistic traits among some adults diagnosed with ADHD, although elevated traits on questionnaires or assessments do not automatically mean someone is autistic.

This is why diagnosis is based on a broader developmental and clinical assessment, not a single behavior or online checklist.

How do you know whether it is autism, ADHD or AuDHD?

There is no single behavior that cleanly answers this question.

Instead, clinicians consider the overall pattern across development and different areas of life.

For ADHD, this includes persistent patterns of inattention and/or hyperactivity-impulsivity that began in childhood and cause impairment across important settings.

For autism, assessment considers lifelong differences in social communication and interaction alongside restricted/repetitive behaviors, interests, routines and sensory characteristics.

When both patterns are present and each independently meets diagnostic criteria, someone may be diagnosed with both autism and ADHD.

NICE recommends considering autism assessment in adults with persistent social communication/interaction differences or restricted/repetitive characteristics when these are associated with difficulties in areas such as relationships, education or employment. A history of another neurodevelopmental condition—including ADHD—is specifically included among the factors clinicians should consider. Read the NICE adult autism guidance.

Similarly, adults presenting with possible ADHD without a childhood diagnosis should receive a full specialist assessment rather than being diagnosed from a screening score alone. NICE explains the adult ADHD assessment pathway here.

What if the hyperactivity is mostly internal?

Adult hyperactivity can be surprisingly invisible.

Someone may sit quietly during a meeting while experiencing:

💭 multiple simultaneous trains of thought
⚡ intense internal restlessness
🧠 a constant need for stimulation
🦵 an urge to move that they consciously suppress
🗣️ an urge to interrupt that they continually inhibit
📱 repeated impulses to switch activities
🔥 intense discomfort during inactivity

Years of social expectations can teach people to suppress visible hyperactivity.

The movement disappears.

The restlessness does not necessarily disappear with it.

This can contribute to ADHD being overlooked in adults who do not match the stereotype of an obviously hyperactive child.

Hyperactive sometimes, exhausted at other times

Another confusing experience is moving between periods of intense activity and periods of very low capacity.

Someone might be:

⚡ restless in the morning
🎯 intensely focused for several hours
🗣️ highly animated during an interesting conversation
🏃 constantly moving during the afternoon

and later:

🔋 completely depleted
🗣️ unable to maintain conversation
🛋️ unable to initiate basic tasks
🎧 extremely sensitive to input
🧠 mentally foggy

This does not necessarily mean the earlier energy was unlimited.

Activity itself consumes capacity.

For neurodivergent adults, sensory processing, masking, executive functioning and attention regulation can all add additional demands.

When those demands accumulate, the contrast between high activation and exhaustion can become dramatic.

If this pattern becomes chronic, it is worth understanding the difference between ordinary tiredness and neurodivergent burnout.

What helps when an autistic person is very active or restless?

The first step is not automatically trying to stop the movement.

Ask what it is doing.

🎧 Sensory regulation?
Provide safer or more effective ways to obtain the sensory input the person needs.

ADHD understimulation?
Movement, stimulation, task variety or appropriate ADHD treatment may help.

😰 Anxiety?
Reducing uncertainty or addressing the source of anxiety may be more useful than suppressing the restlessness.

🔥 Overload?
Reduce demands and sensory input.

🔁 Stimming?
If it is harmless, it may not need fixing at all.

🧠 Attention regulation?
Allowing movement while listening or working may actually improve concentration.

This functional approach is consistent with NICE guidance for autistic adults, which recommends examining factors such as physical health, sensory environments, communication, mental health and changes in routine when understanding behavior.

What if an autistic adult also has ADHD?

Recognizing co-occurring ADHD can matter because it may create support needs that an autism diagnosis alone does not fully explain.

Research involving autistic adults has found that greater co-occurring ADHD symptoms are associated with lower independence in daily living activities and lower self-reported quality of life. Read the study.

ADHD is also treatable.

For people who meet criteria for ADHD as well as autism, current NICE guidance recommends the same ADHD medication choices used for people with ADHD generally, while treatment should of course be individualized and managed by an appropriate clinician. See the NICE ADHD guideline.

Treating ADHD does not treat the core characteristics of autism.

But appropriately addressing co-occurring ADHD may reduce difficulties related to attention regulation, impulsivity or hyperactivity.

Hyperactivity is a clue, not a diagnosis

Seeing an autistic person constantly moving does not tell you why they are moving.

It could reflect:

⚡ ADHD hyperactivity
🔁 autistic stimming
🎧 sensory seeking
😰 anxiety
🔥 overload
❤️ emotional regulation
🧠 attention regulation
🎯 excitement
🔀 several of these at once

That distinction matters because different causes call for different support.

Instead of asking only:

“How do we stop the hyperactivity?”

ask:

“What function is this movement serving?”

Sometimes movement is the difficulty.

Sometimes movement is actually how the person is managing the difficulty.

And sometimes the combination of autism, hyperactivity, attention differences, sensory seeking and conflicting needs points toward a broader question:

Could both autism and ADHD be part of the picture?
